I've noticed recently that when I try to open a file, the "Favorites"
list is no longer included in the Open dialog. Is this a bug, or maybe
something I missed somewhere?

I assume you mean in the open file dialog? If so then click the icon on the
far right, top of the dialog. Your list is now there. :)

You "assume" this because you posted the original question.

People are not as stupid as you need them to be.

Are your favorites also missing from Finder window sidebars?

When you go into Finder preferences > Sidebar, are any of your favorites
checked?

No.

Yes. They seem to have no relation to what shows up in an Open/Save
dialog. I've got things unchecked in Finder Prefs which show up in
Open/Save, but are properly not showing up in Finder windows.

Which app's open dialog is in the above screen shot?

Does the same thing happen in other app's open dialogs, or just this app?

Does the issue occur in another OS X user account?

BBEdit 11.1.3

Happens in Photoshop CC 2015.0.1 too.

I just created a new user account, added a non-standard Favorite in
Finder, and that showed up in BBEdit. This seems to point to some kind
of Preferences file failure. Any idea which one?

Seems others have similar issues. Here are some suggested fixes:
-sidebar-items> OR <http://goo.gl/h8AOR7>

Yep, looks like a known bug in El Capitan.

All you seem capable of doing so far here is insulting others and
trolling otherwise useful threads with trollish posts.

This is from your buddy Snit

"None of you can be honest... you are all pathetic." - Snit
"I do not KF people" - Snit
"Not only do I lie about what others are claiming,
I show evidence from the records".-Snit
"You should take one of my IT classes some day." - Snit

Still want to claim he is honest and honorable? Only people who do are his socks and shills.

Agreed. Snit hasn't been naughty here. Sounds like Steve needs to calm
down a bit. Shit happens on USENET. Grow a thicker skin, you'll be
happier, Steve. No one wants the drama.

Snit is setting you up. He comes into a group pretending to be reasonbale but uses socks and shills and forgeries to attach the group. He killed CSMA and COLA and has now targetted you.

At this moment it's you who is trolling us with your shitty noise-full
and content-free posts, tosspot. We're all telling you to fuck off,
we're not interested. If Snit (or anyone else, for that matter) turns
into a troll he will be dealt with in his turn.

My preference: posts about technology. Even I, long ago, became bored with
the posts about me. I assure you I am not that interesting. :)
