There is a bad one in Game of Thrones, when Hodor finally says "hold the 
door". The translators had not been told of this in advance and they 
could not name the character appropriately from the start, nor do a good 
play of words when we learned the reason of that word.

Lots of virtual ink spilled on this issue.
