As to your survey: I don't know you, you're not supplying anything I need 
and I have no way of knowing what else you'd do with my address if I gave 
it to you. There are a LOT of phishers and spammers out there who'll try 
any form of social engineering to build a saleable address list. How do I 
know you're  not one of them?

All in all I have no reason to give you my e-mail address. In short, if 
you want to run a survey you need to find a way of getting responders 
that doesn't involve them going to the trouble of contacting them first. 

There are free websites that you can use to run surveys. Have you 
considered using them?
