Following up the link that someone posted re this book,

I downloaded and read the whole thing and found it a wonderful insight into 
the way that Chuck was thinking at the time he created Forth. There are 
typo's and grammatical errors but I am sure that, with a bit of editing, 
this would make a nicer to read book. It is just the typo's and grammatical 
constructs that need to be ironed out. The technical content is excellent.
