The engine dropped the head off an exhaust valve in number one cylinder, 
which got punched up into the cylinder head a few times by the piston 
crown until it managed turn sideways and get wedged into the valve 
pocket as can be seen in the photo. All of the damage to the piston was 
concentrated on the exhaust side of the "dome" which is where it did all 
the hammering, and as a result of that it rocked the forged piston over 
in it's bore with bottom of the piston pin boss on the inlet side of the 
engine pushing hard against the bore wall and causing a split.

The cylinder was otherwise undamaged.

I've seen similar things happen before, and it's far from uncommon to 
have component failures such as this in competition engines. In similar 
incidences it's common to bend a connecting rod, but this engine had a 
set of very strong after market 4340 forged H bean rods rods fitted 
which proved to be *way* stronger than what the thinwall casting of the 
block proved to be, and the path of least resistance became the cylinder 
wall. The piston itself, which is a custom forging, survived remarkably 
intact with only some impact marks on the crown and a slight compression 
of the top ring groove jamming the ring on the exhaust side. The engine 
was still able to be turned over, and was in fact driven into the pits 
from the Chase on Conrod straight where the valve let go.

It broke a head off an exhaust valve for whatever reason, and it's not 
an unknown issue with engines that spend a lot of time at high RPM. 
Especially when using custom billet valves like these were. A flaw in 
the material. A failure in the machining process. Who knows?

Krypsis will no doubt give you some long winded jabber about "thermal 
dynamics" and a whole bunch of other irrelevant bullshit that does 
nothing other than try to make him sound important and knowlegable, and 
when he does perhaps you could ask him how his theories *only* applied 
to the valve that failed and not the other 19 that remained perfectly 
fine after it did :)
