It makes a voice band call to the other fax. My being connected to the internet at the same time is independant of the fax. My laptop has a built in fax modem. Winfax is installed and recognizes it as a fax modem, although I use the laptop so seldom that I have never needed to send a fax while using it. There are services that you can email a file to and it gets sent by fax to a number you specify. I have never used these services. This way the fax goes "over the DSL", but in this case, no fax modem is needed.
