OK...it was NOT like TOPS-20, but it was like MULTICS. I worked on it for about eight years, including a large amount of work porting it to XA architecture. But there were not many instances. All disk I/O was unified with paging, and files were mapped into memory (the only way to access them). Other I/O used a related model where the buffers worked like that. 4kB pages because that was determined to be optimal (the hardware pages were grouped to get 4kB ones).
