[Unix] shared memory or memory-mapped files

Pretty much everyone has both these days (different interface to common
function).  What systems supported one but not the other?

Also when did people get the idea to implement 'malloc' in terms of
'mmap' rather than [somewhat awkward] 'sbrk'?

(feel free to comment on other operating systems)
