It appears the machine is having trouble accessing the startup volume.

Run Disk Utility's "Verify Disk" function on the startup volume and see
what it shows.

He should be prepared to replace the hard drive in the machine with a
new one. He may want to consider replacing it with a faster hard drive
or an SSD to increase the speed of the computer. He can restore his data
from his Time Machine backups afterwards.
