There *is* one message that's present in three copies, but that's Eternal September's fault. I don't know what went wrong, but that message was "stuck sending" for quite a bit longer than normal in Agent. I'm assuming that E-S failed to acknowledge receiving the post, but posted it anyway, and Agent made some automatic retries before finally deciding it had successfully posted the article.
